Care includes treatment of routine medical conditions such as Diabetes Mellitus, Hypertension and Thyroid disease, amnesty examinations, anonymous HIV screening and counseling, laboratory services, screening and preventive medical testing education, as well as, minor surgical procedures. Patients with chronic conditions are enrolled in a rigorous program of self-management with a medical support team that educates and encourages their efforts to take control of their disease.
Health Education includes verbal instruction as well as support materials such as videos and pamphlets given by the medical provider and/or the health educator.
Nutritional counseling is made available by a Registered Dietician for Hypertension, Obesity, Diabetes, Prenatal and Pediatric patients as well as any patient with a need for nutritional counseling requested by the Primary Care Provider.
Referrals include networks of community public and private specialties to address the patients' social and medical needs that cannot be met at the facility. Referrals for specialty care include radiology, high-risk pregnancy, mental health, dentistry, x-ray, mammography, social services and Women, Infants and Children (WIC).
